Supplemental Critical Illness

Receive a lump-sum benefit that can help pay out-of-pocket costs if you are diagnosed with a covered critical illness or specified disease on or after your coverage effective date.

Critical Illness Insurance through MetLife will pay you a tax-free, lump-sum cash benefit if you’re diagnosed with a covered condition. The coverage does not replace your medical benefits. You must have medical insurance to enroll in this coverage (although it is not required that your coverage be through Momentive).

Employee, spouse/domestic partner, and child(ren) coverage is available.

Accident Insurance

Accident Insurance can help pay for out-of-pocket costs when you suffer an accident on or after your coverage effective date.

Accident insurance through MetLife provides you with a lump-sum payment for over 150 different covered conditions, such as fractures, dislocations, concussions, eye injuries and more, as well as covered medical services, like an ambulance ride, emergency care and surgery.

There are no waiting periods to satisfy, and any payment will be in addition to any other insurance you may have.

Legal Insurance

Legal coverage isn’t just for the serious issues, it’s for your everyday needs, too. Legal insurance helps you address common situations like creating wills, transferring property or buying a home.

Health Screening Benefit Reminder: After your Supplemental Critical Illness or Group Accident coverage has been in effect for 30 days, MetLife will pay an annual wellness benefit by check per covered person, per calendar year, for taking one of the eligible screening or prevention measures.

Don’t Forget

When you enroll in a Medical option, you will automatically have Critical Illness Insurance that pays a lump sum benefit of up to $2,500 per covered person for each diagnosed condition with a per-person lifetime maximum of $6,000.

Home & Auto

Managed Accounts Service with Fidelity

Fidelity is offering a professionally managed portfolio based on personal investment strategies that align to your unique time horizon, financial situation, and willingness to assume risk. Fidelity will be responsible for daily monitoring and rebalancing, as needed, and providing proactive notifications about investment changes after they are made. Employees are able to enroll and dis-enroll anytime throughout the year. Employees who choose to use the service will be charged an advisory fee based on their 401(k) account balance.
